As the last days of school approach, it becomes necessary to plan for class placements for the next school year.  To help us provide the best educational program for your child(ren) please let us know if you are and are not planning to return to Capri next year.

5th grade parents, please indicate if your student is planning to transition to a CUSD middle school or a middle school outside of CUSD.

If you are not able to complete this form online, please call the front office at 408-364-4260 to let us know if you are or are not planning to return to Capri next year.
