Healthcare Insurance Enrollment

You can enroll your children in Medi-Cal or Healthy Kids healthcare insurance programs on Thursday, May 14, from 9:00 – 11:00 a.m. at the Blackford Elementary School Family Resource Center, 1970 Willow Street in San Jose. Please bring proof of home address, proof of income, and birth certificates for the children. Also, please bring social security cards for the children if available.
